A 40-year-old Latino man presented to the emergency department (ED) after he awoke at 4 AM with blood oozing from his nose. He then began vomiting bright red blood with clots. In the 3 weeks prior, he had presented twice for varying complaints including hyperglycemia to 400 mg/dL, myalgias that impaired walking, and reports that he felt increasingly unwell with fevers, chills, night sweats, headaches, and progressively worsening productive cough. He denied melena, hematochezia, and jaundice.
