Finding Waldo in Narcosis: Identifying Respiratory-Onset Motor Neuron Disease in a Chronic Smoker with Triple-Vessel Coronary Heart Disease

A 65-year-old Malay gentleman presented with a one-week history of exertional dyspnea, associated with orthopnea and bilateral lower limb swelling. He was a former smoker of 66-pack years, with a known history of triple-vessel coronary artery disease.
